Who can apply? Any 18+ year old, US citizen without prescription coverage, within income limits may apply.

    You MUST have a primary care physician. We cannot process any applications for assistance without your doctor's signature. The full cooperation of your physician is required to process the company forms. Please consult your physician for their commitment before you apply to the Life Scripts program.

    Most daily maintenance medications qualify, although Life Scripts does NOT process manufacturer forms for antibiotic, controlled, narcotic or scheduled products.

    The application process to receive initial and refill medication shipments is 6-8 weeks. Refills are available. You may reorder refills at 45 days after receipt of medications.

Income Requirements.

    Your total gross family/household income must meet the requirements listed below. If you fall below the lowest income guideline, you must provide proof of MEDICAID DENIAL from the Department of Heath and Human Services or Department of Job and Family Services, issued by your state or county of residence. This denial letter must accompany your application to Life Scripts Medication Assistance Services.
